IMPORTANT NOTE: As of September 2024, in compliance with the Quebec Government's Language Policy changes, applicants must demonstrate an adequate level of French proficiency (Intermediate/B2 level). Some exceptions apply, please see point "7. Proof of Proficiency in French" at the bottom of this page.

IMPORTANT: The deadline to submit ALL supporting documents is November 3th (11:59 pm - EST). Incomplete applications will be cancelled after that date. This does not apply to the CASPer Test. If you have requested for the results to be sent to us, they will be processed in due time.

1. Transcripts from every post-secondary institution

Academic records from every post-secondary institution are required as part of the supporting documents of your application. ÌýAn unofficial copy of each transcript must beÌýuploaded via your . This includes, but is not limited to:Ìý

  • College or/and university transcripts - even if you are only taking a few courses or have not completed the degree);
  • Exchange or study-abroad sessions;
  • Advanced levels, IBO, French Baccalaureate and AP results if you are using one of these means to meet our science prerequisites.

Please note that unofficial copies of transcripts must include the following information:

-ÌýÌýÌýÌýÌýÌýÌý name of the University

-ÌýÌýÌýÌýÌýÌýÌý name and type of program

-ÌýÌýÌýÌýÌýÌýÌý student's name

-ÌýÌýÌýÌýÌýÌýÌý grades, credits, semesters

Important:ÌýOur ¿´Æ¬ÊÓƵ Office reserves the right to require official academic records at any time during the admissions process, and refuse admission, or rescind any offer of admission made if discrepancies between unofficial and official record(s) are found. Official transcripts sent to ¿´Æ¬ÊÓƵ University become the property of the University and will not be returned or forwarded to other institutions.

Transcripts written in a language other than English or French must be accompanied by a certified translation from a certified Canadian translator. If documents are submitted with a name other than that shown on the application form, proof of name change must be provided in the form of a notarized or certified copy of a marriage certificate or Change-of-name Order.

Current and former ¿´Æ¬ÊÓƵ University students areÌýnotÌýrequired to send transcripts by mail. The ¿´Æ¬ÊÓƵ Office can obtain these documents internally.

2. Academic History Workbook

â–º Download the Workbook provided and complete it with your academic history, including course history. You must save the file on your computer before entering any text in order for the internal links to work.Ìý

â–º Table 2 of the Workbook should be completed with your entire history of higher-learning (e.g. post-secondary, undergraduate, graduate, etc.).Ìý This includes advanced secondary programs such as APs from CollegeBoard, the International Baccalaureate from IBO, CEGEP programs, and the like.Ìý

â–º Table 3 need not contain graduate course history.

â–º Please note some date formats are not compatible with certain operating systems.Ìý Should you receive an error message, please disregard.

â–º If you are using a calculus course to meet half of the physics requirement (3 credits), you do not need to request approval for course substitution. You just need to list this course in Table 4 under 'Physics' in the 'Introductory Basic Sciences' section. It is understood that a calculus course doesn't have a lab component.

Compatibility: The Workbook was created in Microsoft Excel 2016. The desktop version of Excel is recommended; tables are also compatible with the free version of

3. CASPer Test Results

Visit the requirements section of our website for more guidance.

The last possible test dates accepted before the November 1st application deadline are:

  • Test in English: October 16th, 2024
  • Test in French: October 16th, 2024

Register for CASPer at least 3 days before your preferred test date to allow your identification and payment to be processed.

IMPORTANT NOTE: all applicants to our D.M.D. program are required to complete the online assessment test (CASPer) prior to their application deadline. Completion of CASPer is mandatory to maintain admission eligibility. There will be no exceptions.

Please note that the test can only be taken once per admission cycle.

4. Curriculum Vitae and List of Verifiers

5. Proof of Citizenship or Residency Status

Residency documents are reviewed by the ¿´Æ¬ÊÓƵ Office, who will make a determination of residency based on the validity of the documents submitted. Applicants whose documents do not fulfill residency requirements for the applicant category they have selected will be notified and switched to the appropriate category. You can visit this section of our website to confirm if you have selected the correct applicant category.

The final document should be in a single PDF file to upload to the Proof of Citizenship/Perm Res (POCR) item on your Dentistry application checklist via your Slate account.

Note: Residency for tuition fee purposes is reviewed separately by Enrolment Services and is established based on the admitted student’s residency situation as of the start of classes.

International Residents (including USA)

Please submit ONE of the following:

  • Valid Passport (from your country of residence)
  • Birth certificate (in original language with official translation in not in English or French)

NOTE: A valid study permit will be required AFTER acceptance and must be valid throughout the duration of the program. For information concerning immigration matters, please contact and/or the .

Canadian (non-Quebec) Residents

Please submit ONE of the following:

  • Canadian birth certificate
  • Proof of Canadian citizenship or permanent residency
  • Certificate of Indian status
  • Makivik society card
  • Canadian Passport.

Please note that you will be required to submit one of the above documents if admitted to allow ¿´Æ¬ÊÓƵ to establish your Quebec permanent code and tuition rate.

Quebec Residents

Note that only Canadian citizens (including Canadian First Nations and Inuit) and Permanent Residents of Canada can claim Quebec residency. To be considered a Quebec resident, you must meet all the criteria of one of the 14 situations outlined by the Quebec Government. You will need to demonstrate how your Quebec Residency was established from the start. Thereby, situations #2, 3 and 14 cannot be used to prove Quebec Residency for the purpose of admission.

IMPORTANT ► Québec residency status must be fully established (not provisionally) by the application deadline for all those applying to the DMD program. Changes of citizenship or residency after the application deadline will not be considered.

NOTE: If you were born in Quebec, you are automatically considered a resident of Quebec. You will, however, still need to provide a copy of your birth certificate or one of the documents listed on this page.

6. Proof of Proficiency in English (if applicable)

Not all applicants need to provide proof of English proficiency. To determine if you need to provide proof of proficiency in English, please visitÌýthis sectionÌýof our website.

All test results must be reported directly to ¿´Æ¬ÊÓƵ University by the test centre in an electronic format. Candidates' copies of results will not be accepted as proof of English language proficiency. It is the applicant’s responsibility to ensure that the official test results in an official format are forwarded directly by the testing service to ¿´Æ¬ÊÓƵ

7. Proof of Proficiency in French (if applicable)

Proof of French Proficiency (Intermediate B2 Level) will be required for theÌýDMD 2024-2025 application cycle. Your proof of French proficiency must be submitted BEFORE the application deadline.Ìý

If you are an international student, a Black candidate pathway student, or an Indigenous pathway student,Ìýyou can apply without providing proof of French proficiency. If you are accepted however, you will need to obtain French proficiency by your 3rd year before starting clinic. Please understand that the DMD is a heavy program, and learning French at the same time will be difficult. Therefore, we do not suggest you enroll if you have no knowledge in French.Ìý

French-language requirement exemption:

  1. All students who have completed both high school and CEGEP in Quebec (in English or French) will not be required to submit tests or certificates for proof of French-language proficiency. An unofficial copy of proof of high school (transcript or diploma) must be uploaded. The CEGEP transcript is received directly from the Ministry of Education.
  2. Successfully completed at least 1 year of secondary* or post-secondary study at an institution whose official language of instruction is French.
  3. Candidates who wrote CASPer (score used for admission to DMD) in French.

*Please note that only full-time studies completed in French will be accepted; French Immersion schools outside of Quebec are not eligible.

Please note that all documents submitted to ¿´Æ¬ÊÓƵ University in support of an application to be admitted, including, but not limited to transcripts, diplomas and test scores (in paper or electronic format), become the property of ¿´Æ¬ÊÓƵ University and will not be returned to the applicant or forwarded to another institution.
